The first time we write a hard word we write it in bold.

If something is not right at work, you can tell us about it.

When you tell us about the problem, you do not have to tell us your name.

This is called an anonymous report.

Anonymous means we do not know who you are.

We will not contact you after you make an anonymous report.

We will not tell your work what you tell us.

Your report will help us

• find out about workplaces doing the wrong thing
• plan our work.

You can tell us if something about your pay is wrong.

You can tell us if your employer does not follow the rules.

We want to know the name and address of where you work.

We want to know what the business does.

We want to know what you do at work.

We want to know about your problem at work.

The online form is not Easy Read.

Your family, carer or friend can help with the online form.

We will not contact you after you send us the online form.
